Along with the core services developed by Senior Resource Connection, the organization operates additional community based programs that assist seniors/disabled and families in maintaining their independence and dignity.

Community Geriatric Assessment Nurse Program

This service provides Montgomery Co. residents age 60 and older an opportunity for a comprehensive health care assessment and referrals to pertinent other healthcare professionals. Face to face assessments are completed by experienced Registered Nurses from the Visiting Home Care staff. Fees for this service are provided by funding from the Montgomery County Human Service Levy. Care Coordination/Caregiver Support Services Programs

Senior Resource Connection is proud to partner with the Area Agency on Aging PSA2 on our Care Coordination and Caregiver Support Services Programs.  To qualify for these programs you must be 60 years old or older and a Montgomery County resident.  Clients will receive services based upon an individualized care plan that is mutually developed between the client and the Care Manager responsible for the coordination of the designated services.  Senior Resource Connection utilizes existing program networks to identify potential clients who may benefit from the delivery of the services identified by the Care Manager or Case Manager(s). The Care Manager is a Registered Nurse who reviews the information and completes a comprehensive in home assessment using considerations such as income, functional level, etc.  These programs are managed through Senior Resource Connections’ Visiting Homecare Department. 

Clients and their caregivers will receive services based upon an individualized care plan that is mutually developed between the client,their caregiver and the Care  Manager responsible for the coordination of the service delivery.  Additional Senior Resource Connection support services available are Social Services and the Community Geriatric Nurse.

Intakes and referrals are received by a coordination team with experience in scheduling, billing, and community agency collaboration. Care Plans and Service Plans are developed using patient care services, adult day service with transportation, emergency response systems, and home delivered meals to address the identified needs of the client. The Care Manager will provide reassessments, home visits and telephone contact per the assessed case management level.

CaregiverSupportService

Case Management Services

This service allows for the in-home intervention by professional case managers, who assist individuals age 60 years and older in establishing a network of services that promote continued independent living. The program is funded by the Area Agency on Aging PSA2 under Title III of the Older Americans Act. The service is for residents of Montgomery County.

Payee Programs

The Senior Payee Program provides Montgomery County residents, age 60 and older, in need of an independent party to address the handling of their ongoing financial affairs. The program is certified by the Social Security Administration as an authorized representative payee program. All staff has completed authorized criminal background checks. All client monies are maintained in protected accounts at a local FDIC Banking Institution and the program is insured. The payee program works in conjunction with The Adult Protective Service Program and is funded by Title XX. There are no fees to the client associated with this program.

The Severely Mentally Disabled Payee Program provides payee services to residents of Montgomery county, age 18 and older, who have been referred by their mental health case manager.  The program is certified by the Social Security Administration and funded through the ADAMHS Board of Montgomery County. There are no fees to the client associated with this program.

Congregate Dining

This service offered in Montgomery, Greene, Miami and Darke Counties affords individuals age 60 and older the opportunity to have a hot, nutritional breakfast or lunch Monday through Friday while enjoying the company of their peers. All counties have multiple locations.
