Our Community Geriatric Assessment Nurse Program provides Montgomery County residents age 60 and older an opportunity for a comprehensive health care assessment with referrals to pertinent other healthcare professionals. Face to face assessments are completed in the comfort and privacy of the client’s home by experienced Registered Nurses.
