Senior Resource Connection is proud to partner with the Area Agency on Aging PSA2 on our Care Coordination and Caregiver Support Services Programs. To qualify for these programs you must be 60 years old or older and a Montgomery County resident. Clients will receive services based upon an individualized care plan that is mutually developed between the client and the Care Manager responsible for the coordination of the designated services. The Care Manager is a Registered Nurse who reviews the information and completes a comprehensive in home assessment using considerations such as income, functional level, etc. These programs are managed through Senior Resource Connection's Visiting Home Care Department.
