Case Management

This service allows for the in-home intervention by professional case managers, who assist individuals age 60 years and older in establishing a network of services that promote continued independent living. The program is funded by the Area Agency on Aging PSA2 under Title III of the Older Americans Act. The service is for residents of Montgomery County. For more information, please contact us here.

Here are just a few of the services a Case Manager provides to keep clients happy, healthy and safe:

• Assess an older person's overall living situation to identify problems
• Determine unmet needs and assess eligibility for needed services
• Arrange for appropriate services to come into an older person's home
• Monitor in-home workers to make sure the needed care is provided
• Assist with completing applications for benefits (Medical Assistance, Pharmacy Assistance, Food Stamps)
• Provide counseling and support
• Maintain contact with out-of-town relatives
