Unfortunately, the Choice Meals Program is not currently available. We will continue to provide meal delivery, but it will be a variety of what is available each day.
This service provides Montgomery Co. residents age 60 and older an opportunity for a comprehensive health care assessment and referrals to pertinent other healthcare professionals. Face to Face Assessments are done by our experienced RN that works for the Community Geriatric Assessment Nurse Program. Senior Resource Connection is proud to partner with the Area Agency on Aging PSA2 on our Care Coordination and Caregiver Support Services Programs. To qualify for these programs you must be 60 years old or older and a Montgomery County resident. Clients will receive services based upon an individualized care plan that is mutually developed between the client and the Care Manager responsible for the coordination of the designated services. The Care Manager is a Registered Nurse who reviews the information and completes a comprehensive in home assessment using considerations such as income, functional level, etc. These programs are managed through Senior Resource Connection's Visiting Home Care Department.
Intakes and referrals are received by a coordination team with experience in scheduling, billing, and community agency collaboration. Care Plans and Service Plans are developed to help address the identified needs of the client. Services may include:

Senior Resource Connection's Representative Payee Program, established in 1990 and certified by the Social Security Administration, serves over 500 clients in Montgomery County. As the only local nonprofit provider of these services, we estimate 5,000 individuals have been helped since this program began with no fees for this service to our clients. All of our staff are subject to criminal background checks and all client monies are maintained in protected accounts at a local FDIC Banking Institution.
Montgomery County residents, age 60 and older, in need of an independent party to address the handling of their ongoing financial affairs may be eligible for funding under Title XX. This program works in conjunction with Adult Protective Services.
Severely Mentally Disabled residents of Montgomery county, age 18 and older, must be referred by their mental health case manager for funding through the Alcohol, Drug Addiction, and Mental Health Services Board of Montgomery County.
We also partner with The Disability Foundation for those who may have funds set aside in a trust.

This service offered in Montgomery, Greene, Miami and Darke Counties provides individuals, age 60 and older, the opportunity to have a hot, nutritional breakfast or lunch Monday through Friday while enjoying the company of their peers. All counties have multiple locations. There is a suggested donation of $3 per meal, but not required. Some sites require reservations while other sites require pre-qualification. An unkempt home can become overwhelming for our seniors. If the situation gets out of control, it can lead to an unsafe environment in which to live. Not only is it a potential health hazard, with an increased risk of pests, but also a dangerous fire and fall risk. If you or someone you know needs help, there are various funding sources to assist with maintaining a clean and safe environment in an individual's home that are beyond the individual's capability. These services include but are not limited to: moving heavy items to provide a safe ingress and egress, non-routine disposal of garbage, pest control preparation and heavy duty cleaning, which consist of washing walls and ceilings, shampooing carpets, cleaning inside cabinets and refrigerator, removing furniture and installing grab bars.

You rely on your medications to keep you healthy. But complex medication schedules can lead to mistakes: missing doses, taking incorrect amounts, or taking pills at the wrong time. These mistakes could potentially lead to unnecessary doctor or hospital visits, illness, or even death. With the help of a Medication Dispenser, you and your loved ones no longer need to worry if you're taking your medications correctly. The dispenser alerts you with an audible reminder and a flashing light (for hearing impaired individuals) when it's time to take your medicine. At the push of the button the machine will dispense an easy-to-handle plastic cup with your pills inside. If a dose is missed, a caregiver can be notified. They can check on you to make sure you're getting your medications.
How it Works: A nurse or caregiver will load the medication cups and put them in the dispenser. Lifeline will help you program the scheduling times for the machine to dispense. You can load up to 60 doses of medication at once, so if someone takes pills 2 times a day, you can load the machine for 30 days!

What are my rights?
The right to be treated with consideration and respect for personal dignity, autonomy, and privacy;
The right to service in a humane setting which is the least restrictive feasible as defined in the care plan;
The right to be informed of one’s own condition, of proposed or current services, treatment or therapies, including alternatives;
The right to a plan that addresses one’s own physical/mental health, social and economic needs, and that specifies the provision of appropriate and adequate services, as available, either directly or by referral;
The right to periodic review, and reassessment of the care plan;
The right to any appropriate and available agency service, regardless of refusal of one or more other services, treatments, or therapies, or regardless of relapse from earlier treatment in that or another service, unless there is a valid and specific necessity which precludes and/or requires the client’s participation in other services;
The right to be advised of and to refuse observation by techniques such as one-way vision mirrors, tape recorders, television, movies or photographs;
The right to have the opportunity to consult with independent treatment, specialists or legal counsel, at one’s own expense;
The right to confidentiality of communications and of all personally identifying information within the limitations and requirements for disclosure of various funding and/or certifying sources, state or federal statutes, unless release of information is specifically authorized by the client or by the parent or legal guardian of a minor client or by the court-appointed guardian of an adult client;
The right to be informed in advance of the reason(s) for discontinuance of service provision, and to be involved in planning for the consequences of that event;
The right to receive any explanation of the reasons for denial of service;
The right not to be discriminated against in the provision of service on the basis of age, color, ethnicity, marital status, gender identity, language, national origin, physical and mental ability, political affiliation, race, religion, sexual orientation, socio-economic status, veteran status, and other characteristics that make everyone unique;
The right to be informed if there are any charges for services;
The right to exercise rights without reprisal in any form including continued and uncompromised access to service;

For non-medical assistance and companionship, there is nothing better than our Visiting Home Care services. Whether you or a loved one needs assistance with bathing, dressing or meal preparation or just need some extra help in light housekeeping, we can help seniors remain at home when health problems arise. All of our home health care workers have successfully completed approved training programs, are bonded, and have completed authorized criminal background checks. In addition, all home health aides are supervised by Registered Nurses. Visiting Home Care staff are on call 24 hours a day in order to address the ongoing needs of our clients. We have your needs, safety, independence, and self-reliance in mind.
Funding for these services may be provided by the Area Agency on Aging PSA-2, Passport, Comcare, MyCare Ohio, Alzheimer's Association, Veteran's Administration, Greene County Council on Aging, or Private Pay options.
Senior Resource Connection’s Nutrition Program delivers hot or frozen meals on a daily basis to eligible elderly and disabled adults in nine counties throughout the Miami Valley. Food is individually packed in microwavable trays and delivered to each clients home along with fruit, bread and milk to ensure each meal is nutritionally balanced.
More than a meal, our trained drivers provide a friendly and daily personal contact that improves our client’s and their family’s sense of well-being. If no one answers when meals are delivered, our friendly staff will call the clients home and/or emergency contact to ensure the client is safe. Senior’s Best Friend is just one of the special services provided by Senior Resource Connection to homebound seniors and disabled adults in our local communities. This supplemental pet food program is for our clients who are currently receiving Meals-on-Wheels services. Senior’s Best Friend prevents those with limited resources from having to choose between food for themselves or food for their cherished pets. It enables individuals to keep their furry friends healthy and fed while providing a special quality to their lives and saving their pets from an uncertain fate.

How do I file a formal grievance?
Upon enrollment in any of our programs, you will be provided a client rights and grievance policy unique to the program in which you participate;
If you have an unresolved issue with your caregiver, representative payee, meal delivery, or other services, the following procedure should be used:
1. For the most effective resolution, within 30 days of incident, the client (or representing professional) should attempt an informal conversation with any involved staff to directly resolve the complaint.
2. If a resolution is not successful within 10 days, a written complaint should be submitted to the attention of Senior Resource Connection’s Director of the program from which you receive services. A response will follow within 14 days of receipt of the written complaint.
3. If you disagree with the decision of the Director of Payee Services, a formal written appeal can be sent to the President of Senior Resource Connection. A response will follow within 14 days of receipt of the written appeal.
All written documentation should be mailed to Senior Resource Connection, 222 Salem Avenue, Dayton, Ohio 45406.
A neutral meeting place can be scheduled between the hours of 8:30 a.m.-4:00 p.m., Monday-Friday. Meeting place, date and time to be set up with the client through their case manager, team member or case management supervisor.
Timelines for solving the grievance, not to exceed 20 working days from the filing date. The filing date is designated as the date the written grievance is received in hand by the Senior Resource Connection President/Client Rights Officer.
Senior Resource Connection Client Rights Officer will provide written notification and explanation of the resolution to the client or griever, if other than the client.
There are no time limits on filing a grievance.
All relevant information about a grievance shall be provided by Senior Resource Connection upon request to any outside entity with which a client has initiated a grievance.
Copies of the grievance procedure will be available upon request.

What are the meals like?
Hot nutritious and delicious delivered right to your door. All Senior Resource Connection meals provide at least one third of your daily required nutrients and include milk, fruit and bread. Special diet/therapeutic meals are available and include low salt, low sugar, low fat, mechanical, and renal.
Who pays for the Meals-On-Wheels?
Meals-On-Wheels is funded from a variety of sources some of which include: the Area Agency on Aging Title III, United Way, and is a passport provider in all counties noted. The program also works with levy funding programs in all counties in their service area. The Meals-on-wheels program accepts contribution’s and fee for service.
